Requesting Medical Records:

Patients or legal guardians may request copies of medical records by submitting a written request to Starlight Pediatrics.

Requests may be made:

  • By email

  • By phone

  • In person at our office

Medical records will be provided in accordance with Texas law and applicable timelines. Any fees, if applicable, will comply with state regulations.

Artificial Intelligence (AI) Use Disclosure:
